Meperidine, Promethazine falls under SEROTONERGIC DRUGS — the group Psilocybin interacts with.

Theoretically, combining psilocybin with serotonergic drugs might cause additive serotonergic effects.
Psilocybin has serotonin 2A (5-HT2A) receptor agonist activity. Theoretically, combining serotonergic drugs with psilocybin might increase the risk of serotonergic side effects, including serotonin syndrome. However, this interaction has not been reported in humans.

Here's the thing to know about Meprozine or Mepergan: it pairs meperidine (a strong opioid pain reliever) with promethazine. Meperidine stands out because it does more than block pain. It also raises serotonin activity in the brain. Psilocybin works on serotonin too, hitting the 5-HT2A receptor. So putting these two together could, in theory, stack up too much serotonin at once.

When serotonin runs too high, people can get agitated, shaky, sweaty, feverish, or have a racing heart and confusion, which we call serotonin syndrome. To be fair, this is a theoretical concern based on how the drugs work, and it has not actually been reported with this combo. Still, meperidine is one opioid we genuinely worry about here.
