Quercetin interacts with whole categories of medication. Diazepam falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Diazepam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses a liver enzyme called CYP2C9 to help clear diazepam, and high-dose quercetin can slow that enzyme down. When the enzyme works slower, diazepam can hang around longer and build up a bit. That means the calming effects could feel stronger than usual, so you might notice more drowsiness, dizziness, grogginess, or feeling unsteady on your feet.
I'll be honest, this concern is mostly theoretical for diazepam. The real human data we have involves other CYP2C9 drugs, not diazepam itself, and the evidence is on the weaker side. Food amounts of quercetin almost certainly won't matter, but concentrated supplements could. Before adding a quercetin supplement, check with your pharmacist or doctor so they can keep an eye on how you feel.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear diazepam (Valium). Quercetin can nudge that enzyme, but the lab and small human studies point in both directions. In some cases it seems to slow the enzyme, which could let diazepam build up a bit and bring on more drowsiness, dizziness, or grogginess. In other studies it actually sped the enzyme up, which could make diazepam a little less effective.
The good news is this is mostly theoretical and based on weak, mixed evidence. Everyday amounts of quercetin from foods like onions and apples are very unlikely to matter. Concentrated supplements are where I'd be more cautious. If you take diazepam regularly and want to add quercetin, please run it by your pharmacist or doctor first.

Diazepam is used to relieve anxiety and to control agitation caused by alcohol withdrawal. It is also used along with other medications to control muscle spasms and spasticity caused by certain neurological disorders such as cerebral palsy (condition that causes difficulty with movement and balance), paraplegia (inability to move parts of the body), athetosis (abnormal muscle contractions), and stiff-man syndrome (a rare disorder with muscle rigidity and stiffness). Diazepam is also used along with other medications to control seizures. Diazepam is in a class of medications called benzodiazepines. It works by calming abnormal overactivity in the brain.
