Why this applies to Effient

Effient falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the wrinkle with Effient (prasugrel): it's a prodrug, meaning your body has to switch it on before it can work. That activation leans on several enzymes, with CYP3A and CYP2B6 doing most of the heavy lifting, not the CYP2C9 pathway that quercetin is thought to slow down. So the interaction on file, which centers on quercetin blocking CYP2C9, doesn't line up cleanly with how Effient is processed.

On top of that, this concern is mostly theoretical and based on lab work and a small study using different drugs (diclofenac and losartan). For Effient specifically, I wouldn't expect a meaningful change in how well it thins your blood or in bleeding risk. Food amounts of quercetin are very unlikely to matter. Still, since Effient does carry real bleeding risk, run any high-dose quercetin supplement past your pharmacist or doctor first.

Effient also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the wrinkle with Effient (prasugrel): it's a prodrug, meaning it arrives in your body inactive and has to be switched on by your liver enzymes (including CYP3A4) before it can thin your blood. So the usual logic flips. If quercetin slows that enzyme down, you could theoretically activate a little less drug and get slightly weaker clot protection. If it speeds the enzyme up, you might activate a bit more.

Honestly, the evidence here is thin and conflicting (mostly lab and animal data, plus mixed human studies), so this isn't a reason to panic. Food amounts of quercetin almost certainly don't matter. Before starting a concentrated supplement while on Effient, just run it by your pharmacist or doctor, since this drug is protecting your heart.

What Is Prasugrel Used For?

Brand name: Prasugrel

Prasugrel tablets (brand names Effient and Prasugrel) are used to lower the risk of dangerous blood clots — including clots that can block a stent — in people with acute coronary syndrome (ACS), which includes unstable angina (chest pain at rest) and heart attacks (both NSTEMI and STEMI types). It is used specifically in patients who are being treated with a procedure called percutaneous coronary intervention (PCI), in which a catheter is used to open a blocked coronary artery, often with a stent.

Effient has been shown to reduce the combined rate of cardiovascular death, nonfatal heart attack, or nonfatal stroke compared to clopidogrel — with the benefit driven mainly by fewer heart attacks.
