Here's the key thing to know: the lovastatin naturally found in red yeast rice gets cleared from your body by a liver enzyme called CYP3A4. Duvelisib slows that same enzyme down. So when the two are taken together, your body may not break down the lovastatin as quickly, and those statin-like levels can quietly climb higher than expected. That's the direction of concern here, more statin effect, not less.

In real life, higher statin levels can mean muscle aches, weakness, or in rare cases serious muscle breakdown and liver strain. This is a theoretical interaction based on how these drugs behave, not on hard studies, so I don't want to alarm you. But since you're already on cancer therapy, please loop in your oncologist or pharmacist before adding red yeast rice.

What Is Duvelisib Used For?

Brand name: Copiktra

Copiktra (duvelisib) is used in adults with relapsed or refractory chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) — meaning the cancer has returned or stopped responding to treatment — after at least two prior therapies.

Copiktra is also used for relapsed or refractory follicular lymphoma (FL) after at least two prior systemic therapies. The follicular lymphoma approval is based on overall response rate and is considered an accelerated approval; continued approval may depend on results from confirmatory trials. Copiktra is not intended for use as a first- or second-line treatment for any of these cancers.
