Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the simple version. Your birth control pill (the ethinyl estradiol and norgestimate combo found in Ortho Tri-Cyclen and its generics) is processed by your liver, and on rare occasions it can put a little strain on liver health. Red yeast rice naturally contains lovastatin, a statin that can also occasionally bump up liver enzymes or, very rarely, irritate the liver. The concern is purely about stacking two things that each lean on the liver.

I'll be honest with you: this is a theoretical worry based on how these work, not a proven reaction, so don't panic. For most healthy people it won't be a problem. Still, if you notice yellowing skin, dark urine, belly pain, or unusual tiredness, check in promptly. It's worth a quick word with your pharmacist or doctor before combining them, especially if you have any liver issues.

Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Here's the thing to know if you're on Ortho Tri-Cyclen (or a similar pill with ethinyl estradiol and norgestimate) and thinking about red yeast rice. The estrogen part of your birth control can mildly slow down the same liver enzyme, CYP3A4, that breaks down the lovastatin naturally found in red yeast rice. In theory, that could let a bit more of that statin build up in your body.

What might that mean in real life? A higher chance of statin-type side effects, mostly muscle aches or weakness, and rarely liver irritation. Keep in mind this is a theoretical concern based on how the drugs work, not strong proof, and red yeast rice products vary a lot in strength. Before starting it, just run it by your pharmacist or doctor so they can keep an eye on you.
