Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Annovera is a vaginal ring that releases two hormones, segesterone and ethinyl estradiol, to prevent pregnancy. Estrogen-containing birth control can, in rare cases, put extra strain on the liver. Red yeast rice naturally contains lovastatin, a statin that can also irritate the liver in some people. The concern here is simple math: using both at once may add up to a little more stress on your liver than either one alone.
To be clear, this is a theoretical caution based on how these products work, not a proven, common problem. Still, it's worth watching for warning signs like yellowing skin or eyes, dark urine, belly pain on the right side, or unusual tiredness. Before combining them, check with your pharmacist or doctor, who may suggest a liver check.

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Here's the thing to know if you use the Annovera ring (which slowly releases segesterone and ethinyl estradiol) and also take red yeast rice. Your birth control hormones lean on a liver enzyme called CYP3A4. Red yeast rice supplies lovastatin, the same statin found in prescription form, which is also cleared by that very enzyme. So they end up competing for the same pathway.
The practical worry runs in one direction: your hormones could slow down the breakdown of the lovastatin in red yeast rice, nudging its levels up. That theoretically raises the odds of statin-type effects like muscle aches or, rarely, liver irritation. This is based on pharmacology, not real patient reports, so it's a soft signal. Still, run it by your pharmacist before combining them.
