CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Phenobarbital, Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses a liver enzyme called CYP2C9 to help clear phenytoin (Dilantin). In lab studies, rhodiola can slow that enzyme down a bit, and one small human study showed it modestly reduced the breakdown of another CYP2C9 drug. Phenytoin is not a prodrug, so if rhodiola slows its clearance, more could build up in your blood.
That matters because phenytoin has a narrow safety window (small changes can tip you over). Higher levels can cause dizziness, unsteady walking, slurred speech, double vision, or drowsiness. The evidence here is mostly theoretical and limited, so this may never cause a problem for you. Still, please check with your pharmacist or doctor before adding rhodiola, since phenytoin levels are easy to monitor.

CNS DEPRESSANTS

A review of adverse event reports in Poland identified cases of excessive sedation, myoclonus, hypotension, and hallucinations when rhodiola was taken with haloperidol, diazepam, or alprazolam.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with phenobarbital (and to a lesser degree phenytoin): both are central nervous system depressants, and rhodiola may add a little to that calming effect. The concern is mostly theoretical and comes from a handful of case reports where people combined rhodiola with sedating medicines and felt more drowsy than expected. We don't have solid studies on rhodiola plus phenobarbital specifically.
If you took them together, the realistic thing to watch for would be extra sleepiness, dizziness, feeling foggy, or unsteadiness. Phenobarbital is already quite sedating on its own. Since this is rated minor and the evidence is anecdotal, I wouldn't panic, but please run it by your pharmacist or doctor before starting rhodiola, especially since phenytoin levels need careful balancing.
