Rooibos and Aspirin, Caffeine, Codeine Phosphate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Rooibos interacts with whole categories of medication. Aspirin, Caffeine, Codeine Phosphate falls into 3 of those categories, so it can interact with Rooibos in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Aspirin, Caffeine, Codeine Phosphate

Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP1A2 substrates may increase the effects of CYP1A2 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP1A2 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

This combo pill has three parts, and the one that matters for rooibos is the caffeine. Caffeine is cleared from your body mainly by an enzyme called CYP1A2. Lab studies (using concentrated rooibos extract, not plain tea) hint that rooibos might slow that enzyme down a little. If it does, caffeine could hang around longer and feel stronger, meaning you might notice more jitteriness, a racing heart, or trouble sleeping.

Honestly though, this is theoretical and based only on test-tube work, so a cup or two of rooibos tea is very unlikely to cause problems. The aspirin and codeine parts aren't affected here. If you regularly use rooibos extracts, just mention it to your pharmacist.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

2 of 3

Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, taking rooibos with CYP2D6 substrates may increase the effects of CYP2D6 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2D6 enzyme activity.

Here's the wrinkle with this combination pill: the codeine part is a prodrug, meaning your body has to convert it into morphine using an enzyme called CYP2D6 before it can relieve pain. Lab studies hint that rooibos may slow that same enzyme down. If that happened in real life, you'd actually make less morphine, so the codeine might work less well for pain rather than building up.

Honestly though, this is based only on test-tube research using concentrated rooibos extracts, not people drinking the tea. A regular cup or two is very unlikely to cause a noticeable change. If you're using strong rooibos extracts or rely on this medicine for pain control, just mention it to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for this combo pill, which mixes aspirin, caffeine, and codeine. The piece that matters most is the codeine. Lab studies hint that rooibos might slow down a liver enzyme called CYP3A4, and your body uses that enzyme partly to clear codeine. If codeine hangs around a bit longer, you could feel its effects more strongly, think extra drowsiness, dizziness, constipation, or slowed breathing in larger amounts.

Now, the reassuring part: this is only seen in test-tube and animal research, not in real people, and a normal cup or two of rooibos tea is very unlikely to cause trouble. Concentrated rooibos extracts are the bigger question mark. If you use those regularly, just give your pharmacist or doctor a quick heads-up.

From our pharmacist’s overview on the Aspirin, Caffeine, Codeine Phosphate page

“Anacin with Codeine combines aspirin, caffeine, and codeine, and each of the three picks up its own supplement problems, so this is a list worth real caution. Just over half of the 398 ingredients here, 202 of them, can add to aspirin's blood-thinning action and raise bleeding risk.”

About Rooibos

What is Rooibos?

Rooibos
  • Caffeine-free beverage
  • Antioxidant support
  • Heart health
  • Blood sugar support
  • Digestive upset
  • General wellness

Rooibos is a caffeine-free herbal tea from South Africa that is rich in antioxidants and generally considered safe as a beverage. While lab and early studies suggest possible benefits for heart health, blood sugar, and inflammation, strong human evidence is lacking for most medical uses. Enjoy it as a tea, but talk to your pharmacist or doctor before using concentrated extracts for health reasons.
