Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.

This combo pill pairs diltiazem (a calcium channel blocker) with enalapril (an ACE inhibitor) to lower blood pressure. The diltiazem part is partly cleared by a liver enzyme called CYP3A4. In test-tube studies, concentrated rooibos extract slowed that enzyme down. In theory, slower clearance could let diltiazem build up a little, which might nudge blood pressure or heart rate lower than expected. You could feel signs like dizziness, lightheadedness, a slow or pounding pulse, swelling, or unusual tiredness.

Here's the honest part: this is based only on lab work, not real people, so the real-world risk looks small. A cup or two of rooibos tea is very unlikely to cause trouble. If you use strong rooibos extracts or notice those symptoms, check in with your pharmacist or doctor first.

Theoretically, taking rooibos with ACEIs may increase the therapeutic and adverse effects of ACEIs.
Clinical research in healthy adults shows that taking a single dose of rooibos tea, 400 mL orally, inhibits angiotensin-converting enzyme activity.

Your medication is a combo pill that includes enalapril, an ACE inhibitor that lowers blood pressure by blocking the angiotensin-converting enzyme. Interestingly, a small human study found that one cup of rooibos tea (about 400 mL) also nudges that same enzyme downward a bit. So in theory, sipping rooibos alongside this drug could add to its blood-pressure-lowering effect, possibly leaving you a little lightheaded, dizzy, or unusually tired, especially when standing up.

I want to be honest though: this is a theoretical concern based on how things work in the lab, not strong proof of trouble. Enjoying rooibos as an everyday tea is very unlikely to cause problems for most people. Just check with your pharmacist or doctor before using concentrated rooibos extracts, and let them know if you feel dizzy.
