Here's the gist for this combo. Phenobarbital already has strong sedating effects of its own, and sassafras may add a little to that drowsy feeling. Putting the two together could, in theory, leave you more tired, foggy, or unsteady than usual. The phenytoin side of your medication isn't really a sedative, so the main concern here is the phenobarbital piece.

I want to be honest about the strength of this: it's a theoretical concern based on how these things work, not something proven in real patients. That said, I'd steer clear of sassafras anyway, since it contains safrole, which the FDA banned in foods over liver and cancer worries. If you're considering it, please loop in your pharmacist or doctor first.

Phenobarbital, Phenytoin also falls under CYTOCHROME P450 1A2 (CYP1A2) INDUCERS.

In vitro research suggests that cytochrome P450 1A2 (CYP1A2) inducers might increase the cytotoxicity of safrole. So far, this interaction has not been reported in humans. Also, it is not known if CYP1A2 inducers increase the toxicity of sassafras. Inducers of CYP1A2 include omeprazole (Prilosec).

Here's the gist of this one. Both phenobarbital and phenytoin can rev up a liver enzyme called CYP1A2. Sassafras contains safrole, and in test tube and animal studies, when that enzyme is sped up, the body converts safrole into byproducts that appear more toxic to cells, especially the liver. So the worry isn't that your seizure medicines stop working. It's that taking sassafras alongside them could, in theory, make the sassafras itself harder on your liver.

The honest truth is this hasn't been shown in real people, only in lab work, so the evidence is weak. Still, safrole is banned from foods because it can damage the liver and may cause cancer, so I'd skip internal sassafras altogether. If you want the flavor, choose safrole-free products, and check with your pharmacist first.
