Deserpidine, Methyclothiazide falls under THIAZIDE DIURETICS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk of hypokalemia in patients taking thiazide diuretics.
Thiazide diuretics increase urinary potassium excretion. Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common complication of intravenous sodium bicarbonate is hypokalemia.

Your medicine pairs deserpidine with methyclothiazide, a thiazide-type water pill. One thing thiazides do is push your body to dump more potassium in your urine, so your potassium can already run on the low side. Sodium bicarbonate (baking soda) can add to that effect, since taking large amounts has been linked to low potassium too. Stacked together, the worry is hypokalemia, which can show up as muscle weakness, cramps, fatigue, or an irregular heartbeat.

This is a theoretical concern based on how the two work, and it really applies to heavy baking soda use (think tablespoons daily), not an occasional pinch for heartburn. Baking soda is also high in sodium, which can work against your blood pressure goal. Please run any regular antacid use past your pharmacist or doctor first.
