Why this applies to Kanamycin

Kanamycin falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk for hypokalemia in patients receiving aminoglycosides.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, when administered intravenously, the most common complication of sodium bicarbonate is hypokalemia. Nephrotoxicity caused by aminoglycosides may lead to increased urinary losses of various electrolytes, including potassium.

Here's the gist for kanamycin (Kantrex), an injectable aminoglycoside antibiotic. Both kanamycin and large amounts of baking soda can independently lower your potassium level. Kanamycin can be tough on the kidneys, and that can cause you to lose potassium and other minerals in your urine. Adding a lot of sodium bicarbonate, which on its own has been tied to low potassium, may stack on top of that.

I want to be honest: this is a theoretical concern based on how these two behave, not strong proof. And we're talking about heavy baking soda use (think spoonfuls daily), not the occasional pinch for heartburn. Low potassium can show up as muscle cramps, weakness, or an irregular heartbeat. Since kanamycin is given under medical supervision, just let your care team know if you use baking soda regularly.
