Mephenytoin falls under CNS DEPRESSANTS — the group Spearmint interacts with.
Animal research suggests that (-)-carvone, a major constituent of spearmint, has sedative effects. However, in humans, chewing spearmint-flavored gum induced arousal effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Mephenytoin (Mesantoin) is an older seizure medicine, and like many antiseizure drugs it can already make you feel drowsy, foggy, or unsteady. The concern here is simply that spearmint might add a little to that calming effect, theoretically leaving you more sleepy than usual. To be honest, this is a very soft worry. It comes from animal studies on carvone, one of spearmint's scent compounds, and in people, spearmint gum actually seemed to perk folks up rather than calm them down.
Bottom line: the tea, gum, or a sprig in your food almost certainly won't move the needle. Concentrated spearmint supplements are less studied, so if you plan to take those daily, run it past your pharmacist or doctor first and don't change your mephenytoin on your own.

Animal research suggests that drinking spearmint tea for 30 days can increase markers of liver damage, including aspartate aminotransferase (AST) and alanine aminotransferase (ALT), and cause liver degeneration and necrosis, in a dose-dependent manner. This effect has not been reported in humans.

Here's the gist for mephenytoin (Mesantoin), an older seizure medicine. Like many anti-seizure drugs, it can put some stress on your liver. The concern with spearmint is simply about adding to that. This isn't about spearmint changing how mephenytoin works or how strong it is. It's that both could nudge your liver in the same direction.
Honestly, the evidence here is thin. It comes from animal studies where high amounts of spearmint raised liver enzymes (AST and ALT). This hasn't actually been seen in people. A little spearmint in food or tea is very unlikely to be a problem. Concentrated spearmint supplements are where I'd pause. If you take mephenytoin, check with your pharmacist or doctor before adding spearmint capsules, and watch for yellowing skin, dark urine, or belly pain.
