Acetaminophen, Codeine, Doxylamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

Here's the honest picture with Mersyndol (acetaminophen, codeine, and doxylamine) and sulforaphane. This interaction is filed under CYP1A2, but it's really a theoretical concern pulled from lab and animal studies, not something seen in actual people. So I wouldn't lose sleep over it.

The trickier point is that the data pulls in two directions: sulforaphane may slow CYP1A2 in some studies, while its precursor may speed it up. With codeine specifically (a prodrug that the body has to activate, mostly through a different enzyme called CYP2D6), the real-world effect on its pain relief or drowsiness is very uncertain. Eating broccoli is no worry at all. If you're taking a concentrated sulforaphane supplement, just run it by your pharmacist or doctor first.

Here's the interesting wrinkle with Mersyndol. One enzyme called CYP2E1 helps process the acetaminophen part. In lab dish studies, sulforaphane appears to slow that enzyme down. Now, this might actually be a good thing in theory. CYP2E1 is the same pathway that turns a small fraction of acetaminophen into a toxic byproduct that can stress the liver, so blocking it could mean slightly less of that harmful breakdown. But honestly, the evidence here is very thin. It comes from test-tube work only and has never been seen play out in real people.

The everyday takeaway: eating broccoli is no concern at all. If you're taking a concentrated sulforaphane supplement regularly, just mention it to your pharmacist, especially since acetaminophen and the liver are worth being careful about.
