Ivosidenib falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the interaction actually runs in the other direction from what you might expect. It is the thyroid extract whose absorption can be thrown off, not your cancer medicine. Thyroid hormones (T3 and T4) are picky about timing, and taking them too close to food or other pills can blunt how much your body soaks up. So if you swallow your thyroid product right alongside your ivosidenib (Tibsovo), the thyroid part may work less reliably, which can leave you feeling tired, cold, or sluggish.

This is a minor, well-documented absorption issue, usually fixed by separating them in time (thyroid hormone on an empty stomach, about 30 to 60 minutes before other things). Since thyroid extract doses are unpredictable, please loop in your oncology team or pharmacist before mixing the two.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
