Turmeric and Lomustine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ALKYLATING AGENTS

Why this applies to Lomustine

Lomustine falls under ALKYLATING AGENTS — the group Turmeric interacts with.

Turmeric has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals. However, research is conflicting.
In vitro research suggests that curcumin, a constituent of turmeric, inhibits mechlorethamine-induced apoptosis of breast cancer cells by up to 70%. Also, animal research shows that curcumin inhibits cyclophosphamide-induced tumor regression. However, some in vitro research shows that curcumin does not affect the apoptosis capacity of etoposide. Also, other laboratory research suggests that curcumin might augment the cytotoxic effects of alkylating agents. Reasons for the discrepancies may relate to the dose of curcumin and the specific chemotherapeutic agent. Lower doses of curcumin might have antioxidant effects while higher doses might have pro-oxidant effects. More evidence is needed to determine what effect, if any, turmeric might have on alkylating agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this one. Lomustine fights brain tumors and Hodgkin's lymphoma partly by creating free radicals that damage cancer cells. Turmeric's main compound, curcumin, is an antioxidant, so the theory is that it could mop up some of those free radicals and possibly blunt how well your chemo works. That's the concern worth knowing about.

But I want to be honest about how shaky this is. The evidence comes only from lab and animal studies, and the results actually conflict. Low doses of curcumin may act as an antioxidant, while higher doses might do the opposite. We simply don't know the real effect in people taking Lomustine. With something this important, please don't add a turmeric supplement without first checking with your oncology team. Food-amount turmeric in cooking is far less of a worry than concentrated capsules.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Lomustine page

“One capsule of lomustine (Ceenu) often carries a whole chemotherapy cycle, so supplements that keep it from being absorbed, or that blunt its cancer-fighting action, deserve more respect here than with most drugs. The lone major-rated interaction, activated charcoal, can bind lomustine in the gut and sharply cut how much reaches the body.”

About Turmeric

What is Turmeric?

Turmeric
  • Joint pain and arthritis
  • Inflammation
  • Digestive upset
  • Antioxidant support
  • Skin conditions

Turmeric is a popular spice whose main active compounds, curcuminoids, are studied mostly for inflammation and joint pain. Some research is promising, but quality is mixed and curcumin is poorly absorbed on its own. For most people, dietary amounts are safe, but talk to your pharmacist or doctor before taking concentrated supplements, especially if you are pregnant, nursing, or have a medical condition.
