Here's the concern with pairing turmeric and Pralsetinib (Gavreto). This isn't about turmeric changing how Pralsetinib works in your body. Instead, both can be tough on the liver. Pralsetinib is a cancer medicine that's known to raise liver enzymes and, in some people, cause real liver injury. Turmeric, mainly in concentrated, high-dose, or highly absorbable supplement forms, has been linked to liver problems too. Stacking two liver stressors could add up.

To be honest, this is mostly a theoretical, anecdotal worry, not something proven for this exact combo. Everyday cooking amounts of turmeric are very unlikely to matter. But since you're already on a drug your team monitors with liver blood tests, please check with your oncologist or pharmacist before adding a turmeric supplement.

Pralsetinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the gist: your cancer drug pralsetinib (Gavreto) is cleared by a liver enzyme called CYP3A4, and turmeric's active compounds (curcumin) can nudge that enzyme. In lab and animal studies, turmeric mostly blocks CYP3A4, which could let pralsetinib build up in your blood. That might mean stronger side effects like high blood pressure, fatigue, swelling, low blood counts, or constipation. A few case reports tie turmeric to extra toxicity with similar cancer drugs.

Now, the honest part: this is mostly test-tube and animal evidence, not strong human proof, and large supplement doses matter more than turmeric sprinkled on food. Please don't start or stop anything on your own. Just loop in your oncology pharmacist or doctor before using concentrated turmeric capsules.

Here's the gist with turmeric and your pralsetinib (Gavreto). Pralsetinib is moved around by a little cellular pump called P-glycoprotein, which helps control how much of the drug gets absorbed. Lab and animal studies suggest curcumin, the active part of turmeric, may slow that pump down. In theory, that could let a bit more pralsetinib into your system, which might nudge up side effects like tiredness, constipation, high blood pressure, swelling, or changes in your blood counts.

I want to be honest about the strength here: this is theoretical, based only on lab and animal data, and it's rated minor. Everyday turmeric in cooking is very unlikely to matter. Concentrated curcumin supplements are a different story, so please run those by your oncology team or pharmacist before starting.

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
