Vinpocetine and Tinzaparin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Tinzaparin

Tinzaparin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Vinpocetine interacts with.

Vinpocetine might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
Clinical research shows that vinpocetine decreases red blood cell aggregation, as well as plasma and whole blood viscosity. This effect has been seen with intravenous vinpocetine 1 mg/kg and oral vinpocetine 30 mg daily. Vinpocetine also seems to have antiplatelet effects.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind: Tinzaparin (Innohep) is already a blood thinner, a low-molecular-weight heparin you inject to prevent or treat clots. Vinpocetine isn't broken down by tinzaparin's pathways, but research suggests it can make blood less sticky on its own. It seems to lower platelet clumping and thin the blood a bit. Stack that on top of tinzaparin and you may be nudging your bleeding risk higher.

What that can look like is easy bruising, nosebleeds, bleeding gums, or longer bleeding from small cuts. More serious signs would be blood in urine or stool, or unusual headaches. The evidence here is fairly limited, so please don't panic or stop anything. Just loop in your pharmacist or doctor before combining them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

“Someone giving themselves tinzaparin (Innohep) injections is usually recovering from a blood clot in the leg or lung, or trying to prevent one, and that is exactly why this list of 218 supplements deserves real seriousness rather than a shrug. Nearly all of them, 202 in fact, push in one direction: extra bleeding risk on top of a drug…”
