Why this applies to Piroxicam

Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Vinpocetine interacts with.

Theoretically, vinpocetine might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that vinpocetine weakly inhibits CYP2C9. However, this effect has not been reported in humans.

Here's the basic idea: your body uses a liver enzyme called CYP2C9 to break down piroxicam (Feldene). In the test tube, vinpocetine appears to weakly slow that same enzyme down. If that happened in real life, piroxicam could clear a bit more slowly and build up, which might make its effects and side effects more noticeable. With an NSAID like piroxicam, that can mean things like stomach upset, heartburn, an increased risk of stomach bleeding or ulcers, fluid retention, or strain on the kidneys.

The honest part: this is only a theoretical concern from lab studies, and it has not actually been seen in people. So I wouldn't lose sleep over it, but since both can affect the gut, it's worth a quick chat with your pharmacist or doctor before combining them.

Vinpocetine might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
Clinical research shows that vinpocetine decreases red blood cell aggregation, as well as plasma and whole blood viscosity. This effect has been seen with intravenous vinpocetine 1 mg/kg and oral vinpocetine 30 mg daily. Vinpocetine also seems to have antiplatelet effects.

Here's the thing to keep in mind with piroxicam and vinpocetine. Piroxicam is an NSAID, and one of its known downsides is that it can irritate the stomach lining and make it harder for your blood to clot normally. That's why it can sometimes cause bruising, stomach upset, or even bleeding in the digestive tract.

Vinpocetine appears to have its own mild blood-thinning effect, making platelets a little less sticky. So stacking the two together could nudge your bleeding risk up a bit more than piroxicam alone. Watch for things like unusual bruising, black or tarry stools, or stomach pain. The evidence here is moderate, not alarming, but it's worth a quick chat with your pharmacist or doctor before combining them.
