Dirithromycin falls under MACROLIDE ANTIBIOTICS — the group Wahoo interacts with.

Here's the short version: Wahoo naturally contains cardiac glycosides (heart-active compounds, similar in action to digoxin). Dirithromycin (Dynabac) is a macrolide antibiotic, and macrolides can slow how the body clears these heart compounds. The worry is that the two together could let those heart-active substances build up, raising the chance of cardiac glycoside toxicity, things like nausea, a slow or irregular heartbeat, or dizziness.
I want to be honest about the strength of this: it's theoretical, based on how the drugs work rather than real-world reports. But Wahoo is genuinely poisonous with no proven benefit, so the safest move is simply not to use it.
