Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.

Overview

Bifidobacterium longum is a type of beneficial bacteria that lives naturally in the human digestive tract. It is one of the first bacteria to colonize the gut of newborns, especially breastfed babies, and it remains an important part of a healthy gut microbiome throughout life.

As a supplement, B. longum is grown in controlled conditions and sold as a probiotic. It comes alone or, more often, combined with other probiotic strains. You can find it in capsules, powders, sachets, and some fermented foods or dairy products.

People take B. longum mainly to support digestive health, but it is also marketed for immune support and, more recently, for mood and stress through the so-called 'gut-brain axis.'

How it works

As a probiotic, Bifidobacterium longum is thought to work by adding helpful bacteria to the gut. Proposed mechanisms include crowding out harmful bacteria, producing short-chain fatty acids that nourish the gut lining, and helping keep the gut barrier healthy.

It may also interact with the immune system in the gut and influence signals that travel between the gut and the brain. Much of this understanding comes from laboratory and animal studies, so how strongly these effects apply to people is still being worked out.

Safety & precautions

For most healthy people, Bifidobacterium longum is considered generally safe and is widely used in foods and supplements. However, some groups should be more careful.

People with weakened immune systems, those who are critically ill, who have central venous catheters, or who have recently had major surgery should talk to a doctor first, because rare serious infections from probiotics have been reported in such cases.

For pregnancy and breastfeeding, probiotics are often considered low risk, but high-quality safety data are limited. Because of this, we recommend using B. longum only with the guidance of your doctor during these times.

Side effects

Side effects are usually mild. The most common ones include gas, bloating, and mild stomach discomfort, which often settle as your body adjusts.

Serious side effects are rare. In very rare cases, mainly in people who are seriously ill or immune-compromised, probiotic bacteria have been linked to bloodstream or other infections. Stop the product and seek medical care if you develop a high fever, severe abdominal pain, or feel very unwell after starting it.

Cancer Immunologic

There have been rare cases of Bifidobacterium bacteremia in critically ill infant and adult patients. Various cases of Bifidobacterium longum bacteremia, sometimes presenting as sepsis, have occurred in preterm infants using probiotics. In one case report, a 15-month-old female infant with congenital heart defects and recent surgery to replace a mechanical heart valve developed Bifidobacterium sepsis after being treated with IV antibiotics, extracorporeal membrane oxygenation (ECMO), and oral probiotics containing B. longum. It was thought that ECMO contributed to translocation of bifidobacteria from the gut and into the blood. In 5 cases, very-low birthweight preterm infants developed B. longum bacteremia following the use of a specific probiotic product providing B. longum and Lactobacillus acidophilus (Infloran) for the prevention of necrotizing enterocolitis; antibiotic treatment was required in at least some of the cases. Cases of sepsis related to B. longum have also occurred in adults; however, association with supplementation is unlikely. In one case, sepsis with B. longum occurred following acupuncture. This was likely due to needle contamination and not to supplementation. In another case, a 71-year-old male with liver cirrhosis and prostate cancer developed B. longum lumbar vertebrodiscitis. The source was thought to be translocation from the intestine. A 42-year-old male developed B. longum peritonitis secondary to intestinal perforation.

Dosing

There is no single established 'correct' dose for Bifidobacterium longum. Probiotic doses are usually measured in colony-forming units (CFU), and products vary widely in how many CFU they contain and which strains they include.
