Moderate evidence
The evidence for artemisinin is mixed and incomplete. It's possibly effective for hay fever (allergic rhinitis), though the data aren't rock-solid.
For nonalcoholic fatty liver disease, malaria, and osteoarthritis, there isn't enough reliable evidence to say whether it works or doesn't. We don't have a clear answer on those uses from the data we hold.
How safe is it?
Well-documented data
Artemisinin is generally well tolerated when taken by mouth. The most common side effect is nausea and vomiting.
Rarely, it can cause liver damage (hepatotoxicity) — one documented case involved liver inflammation after 10 days of high-dose use, though symptoms resolved within 2 weeks of stopping. A mild allergic reaction (rash and cough) has been reported once, and some people allergic to Sweet Annie have felt tongue numbness or throat itching when taking it under the tongue.
The safety data advises against using this product during pregnancy because of possible harm to the developing baby. There isn't enough safety information to recommend it while breastfeeding — talk with your doctor or pharmacist for personalized advice if you're nursing.
Because quality and dose vary widely across products and it should never replace proven treatments, this is generally used only short-term.

Artemisinin
Cytochrome P450 2B6 (Cyp2B6) Substrates
Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP2B6.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP2B6, possibly increasing CYP2B6 activity by 1.6-fold. However, Sweet Annie extract seems to inhibit the activity of CYP2B6 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP2B6 substrates.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.
