MicroBiome-18 contains 18 active probiotic strains — a mix of Bifidobacterium species (bifidum, animalis lactis, longum, and breve), Lactobacillus species (salivarius, plantarum, paracasei, acidophilus, casei, fermentum, bulgaricus, gasseri, reuteri, and rhamnosus), Streptococcus thermophilus, Lactococcus lactis, and Bacillus subtilis DE111. These are live microorganisms meant to support your gut bacteria balance.
The inactive ingredients are rice, hypromellose, water, gellan gum, rice bran, and tapioca dextrin — these are capsule material and fillers.
Does it work?
Moderate evidence
The evidence for most of these strains is still being studied. Bifidobacterium bifidum and Lactobacillus acidophilus are possibly effective for irritable bowel syndrome (IBS); L. acidophilus is also possibly effective for Helicobacter pylori, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum may help with respiratory tract infections. Bacillus subtilis is possibly effective for antibiotic-associated diarrhea.
For most other conditions listed in the research — allergies, anxiety, asthma, constipation, and others — the evidence isn't strong enough yet to rate these ingredients as effective. If you're taking this for a specific health concern, talk with your doctor about whether the strains in this product have any real support for that use.
How safe is it?
Well-documented data
These probiotics are generally well tolerated in healthy people, though bloating and flatulence have been reported with probiotic use in general (though not specifically tied to all strains here). A small number of trial participants reported mild gastrointestinal side effects like abdominal discomfort and flatulence.
In rare cases, rashes and itching were noted in one trial, though it wasn't clear they came from the probiotics. People with weakened immune systems — including those who are critically ill, have central lines, or are in the ICU — should be cautious; there are rare reports of these bacteria entering the bloodstream in such patients.
If you're seriously ill or immunocompromised, check with your doctor before starting. For pregnancy, Bifidobacterium bifidum, Lactobacillus acidophilus, Streptococcus thermophilus, and Bifidobacterium longum are rated possibly safe, but safety data are limited.
For Lactobacillus bulgaricus, Lactobacillus gasseri, Bifidobacterium breve, and Bacillus subtilis, there isn't enough data on file to know either way — talk with your doctor or pharmacist for personalized advice about use during pregnancy or breastfeeding.

Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
Lactobacillus acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium longum
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
