HelloPharmacist Scorecard of Nexabiotic Chewable Probiotics Grape Flavor by DrFormulas
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
This product contains 20 active probiotic ingredients—live beneficial bacterial and yeast strains meant to support digestive health. The main species include Lactobacillus and Bifidobacterium strains (which are commonly used probiotics), Bacillus coagulans and Bacillus subtilis (soil-based organisms), Saccharomyces boulardii (a beneficial yeast), and Streptococcus thermophilus.
The product also includes inactive ingredients like dextrose, xylitol, microcrystalline cellulose, grape flavor, vegetable magnesium stearate, and citric acid.
Does it work?
Moderate evidence
The evidence for these probiotics varies widely by strain and by condition. Saccharomyces boulardii is possibly effective for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, and Clostridioides difficile infection.
Lactobacillus acidophilus is possibly effective for irritable bowel syndrome, Helicobacter pylori, antibiotic-associated diarrhea, and bacterial vaginosis. Bacillus coagulans is possibly effective for constipation and irritable bowel syndrome.
Bifidobacterium bifidum is possibly effective for irritable bowel syndrome and respiratory tract infections. For most other conditions listed in the research—such as anxiety, cognitive decline, asthma, athletic performance, and various other uses—the data we hold shows insufficient evidence to rate effectiveness.
Overall, this product is best supported for gastrointestinal concerns like diarrhea and IBS, though even there the evidence is "possibly" rather than definitively effective.
How safe is it?
Well-documented data
These live probiotic strains are generally well tolerated in healthy adults and children. The most common side effects from probiotic use are mild gastrointestinal complaints—bloating, flatulence, abdominal discomfort, and diarrhea—though these are uncommon.
Rarely, some probiotic strains have been associated with serious infections (bacteremia, sepsis, abscess) in patients who are critically ill, immunocompromised, or have central venous catheters, though such cases are not expected in otherwise healthy people. For pregnancy, Lactobacillus acidophilus, Bifidobacterium bifidum, and Streptococcus thermophilus are rated possibly safe, but safety data for pregnancy and lactation for the other ingredients are not on file.
Talk with your doctor or pharmacist before using this product during pregnancy or while breastfeeding.
Meds to double-check
Moderate interaction found
Check with your doctor or pharmacist before taking this product if you use antibiotic drugs—they might reduce how well these probiotics work, and you may need to space doses apart. Antifungal medications also interact with the Saccharomyces boulardii in this blend.
These are the main medication concerns the data shows for the ingredients we could check.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This probiotic blend is generally well tolerated and may help with gastrointestinal issues like diarrhea or irritable bowel syndrome, though the evidence is modest. If you take antibiotics or antifungal medications, you'll need to separate doses from this product—check the specific timing with your pharmacist or doctor.
People who are critically ill or have weakened immune systems should discuss this with their healthcare provider before starting. Run your exact medications through the checker on this page before adding this product to your routine.

Nexabiotic Chewable Probiotics Grape Flavor by DrFormulas Drug Interactions
HelloPharmacist Interaction Report
Nexabiotic Chewable Probiotics Grape Flavor by DrFormulas contains 20 probiotic strains, several of which interact with medications.
Through its Lactobacillus acidophilus, Bifidobacterium longum, Bacillus subtilis (DE111), Bacillus coagulans, Bifidobacterium breve, Saccharomyces boulardii, Bifidobacterium bifidum, Lactobacillus helveticus, and Lactobacillus gasseri content, this product may interact with antibiotics and antifungals. The most serious concern is a Moderate interaction: antibiotic drugs might kill the live organisms in these probiotic strains, which could decrease how well the probiotics work.
Saccharomyces boulardii also has a Minor interaction with antifungals (which might reduce its effectiveness). Altogether, these interactions span 222 individual medications.
Read the full breakdown — every affected drug type, severity by severity
The antibiotic interaction is the same across all the Lactobacillus and Bifidobacterium strains in this product. Because these are live organisms, taking them at the same time as antibiotics may kill a significant number of them.
The data suggests separating doses of antibiotics and this probiotic product. Saccharomyces boulardii, a live yeast ingredient, has a similar but Minor interaction with antifungal drugs.
We could not check several other ingredients in this product—Lactobacillus plantarum, Lactobacillus paracasei, Lactobacillus brevis, Bifidobacterium animalis lactis, Lactobacillus rhamnosus, Lactobacillus salivarius, Lactobacillus casei, Lactococcus lactis, and Lactobacillus rhamnosus LB3—because we hold no interaction data for them. One ingredient, Streptococcus thermophilus, was checked and shows no interactions documented in our data.
Before starting this product, run it through the medication checker on this page with your exact prescriptions.

Lactobacillus acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium longum
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
