Testolone is an unapproved experimental drug with reported serious risks and should not be used. It is possibly unsafe overall.

Common adverse effects include decreased appetite, dehydration, elevated liver enzymes, vomiting, and weight loss. Rare but serious effects include acute heart muscle inflammation (myocarditis), low blood phosphate (hypophosphatemia), and liver injury.

A case of myocarditis was reported in a 32-year-old man using the product; he presented with shortness of breath and fever and required hospitalization. Abnormal hair growth and acne occurred in individual users.

Testolone is hormonally active and unstudied in pregnancy—do not use if pregnant. No safety data exist for breastfeeding; avoid completely if breastfeeding.

Theoretically, concomitant use might have additive hepatotoxic effects.
In a case report, a 52-year-old male was diagnosed with liver injury thought to be related to the use of products containing testolone. Also, elevated liver enzymes were commonly reported in a dose escalation study of testolone.
