5-htp and Amoxapine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Amoxapine

Amoxapine falls under SEROTONERGIC DRUGS — the group 5-htp interacts with.

Combining serotonergic drugs with 5-HTP might cause additive serotonergic effects.
5-HTP can increase serotonin levels and cause serotonergic effects. Theoretically, combining serotonergic drugs with 5-HTP might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. However, serotonin syndrome with 5-HTP has not yet been reported in humans. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using 5-HTP with serotonergic drugs.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about pairing 5-HTP with amoxapine: your antidepressant already works by boosting serotonin and other brain chemicals to lift your mood. Since 5-HTP is a direct building block your body turns into serotonin, taking both at once can stack serotonin on top of serotonin. In theory, that raises the chance of too much, which can show up as agitation, sweating, shivering, a racing heartbeat, muscle twitching, or in rare cases a serious reaction called serotonin syndrome.

I want to be straight with you though: this concern is based on how these substances work, not on confirmed cases, and serotonin syndrome from 5-HTP hasn't actually been reported in people. Still, amoxapine is a real serotonin-boosting drug, so please don't add 5-HTP without checking with your doctor or pharmacist first.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Amoxapine page

“Amoxapine (Asendin) is an older tricyclic antidepressant, and its supplement list deserves real care. Three of the 68 ingredients on file are rated major: activated charcoal can soak up the drug before your body gets it, lithium raises the risk of serotonin syndrome (a dangerous serotonin overload), and scopolia piles onto amoxapine's…”
