Here's the gist for this pairing. Avacopan can act as a mild blocker of an enzyme called CYP3A4, and that same enzyme helps clear modafinil, the active stimulant your liver makes from adrafinil. So in theory, taking Avacopan alongside adrafinil could slow down how fast modafinil is broken down, letting it build up a bit. That might mean stronger or longer-lasting stimulant effects, things like trouble sleeping, a racing heart, jitteriness, headache, or higher blood pressure.

I'll be honest though, this is a theoretical concern based on how the drugs work, not on real-world reports. Adrafinil itself is unregulated and rough on the liver, so I'd talk it over with your doctor or pharmacist before combining the two, especially since you're managing a serious blood vessel condition.

Here's the gist for Avacopan (Tavneos): your liver relies on an enzyme called CYP3A4 to break it down. Adrafinil turns into modafinil in your body, and modafinil can gently speed up that same enzyme. If CYP3A4 works faster, it can clear Avacopan a little quicker, which may lower the amount in your blood and make it less effective at calming the immune attack on your blood vessels.

I want to be honest about the strength of this: it's theoretical, based on how these compounds behave rather than real patient reports, and modafinil is only a weak booster of that enzyme. What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
