Why this applies to Futibatinib
Futibatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing these two. Futibatinib (Lytgobi) is a targeted cancer medicine used for certain bile duct cancers, and your liver clears it using an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that faster clearance could lower the amount of futibatinib in your blood, which might weaken how well it fights the cancer.
I want to be honest about the strength of this: it's theoretical, based on how these drugs behave rather than real-world reports, and modafinil is only a weak inducer. Still, with a cancer drug you don't want any guessing. Please loop in your oncologist or pharmacist before combining them so nothing undercuts your treatment.

Futibatinib (brand name Lytgobi) treats adults with intrahepatic cholangiocarcinoma, a cancer of the bile ducts inside the liver. It is for cancer that has already been treated, can't be removed by surgery, and is locally advanced or has spread.
The tumor must have an FGFR2 gene fusion or other rearrangement. This use is under accelerated approval, so continued approval may depend on confirmatory trials.
