Istradefylline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for Istradefylline (Nourianz). Your liver clears this medicine using an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can gently speed that enzyme up. If the enzyme works faster, it can clear istradefylline a little quicker, which may leave less of it in your system.

In plain terms, your Nourianz could become a bit less effective, so those Parkinson's "off" episodes (trouble moving, walking, or speaking as your levodopa wears off) might not be controlled as well. I'll be honest though: this is a theoretical concern based on how the drugs behave, not proven cases, and modafinil's effect on the enzyme is weak. Adrafinil is also unregulated and tough on the liver, so please run it past your doctor or pharmacist before combining them.
