Here's the situation with these two together. Ketoconazole is a strong blocker of a liver enzyme called CYP3A4, and that same enzyme helps clear modafinil, the active stimulant your body makes from adrafinil. So when ketoconazole slows that enzyme down, modafinil can hang around longer and build up. In plain terms, you may feel more of modafinil's effects: extra jitteriness, trouble sleeping, a faster or pounding heartbeat, headache, anxiety, or stomach upset.

I'll be honest, this concern is theoretical and based on how the drugs work, not on solid studies, so it may never cause a problem for you. Still, both adrafinil and ketoconazole can stress the liver, which is another reason to be cautious. Please check with your pharmacist or doctor before combining them.

Here's the gist with this pairing. Adrafinil turns into modafinil in your liver, and modafinil can gently speed up an enzyme called CYP3A4. That enzyme is one of the pathways your body uses to break down ketoconazole. So in theory, if you take adrafinil, your liver might clear ketoconazole a little faster, leaving lower levels in your blood. For an antifungal, lower levels can mean it works less well against the infection you're trying to treat.

I'll be honest, this is a theoretical concern based on how the drugs behave, not something proven in real patients, and modafinil is only a weak booster of that enzyme. Still, before combining these, check in with your pharmacist or doctor so your infection stays properly treated.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
