Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.

Here's the gist for this pairing. Lonafarnib (Zokinvy) is a known blocker of the CYP3A4 enzyme. Adrafinil turns into modafinil in your liver, and modafinil is cleared by that same CYP3A4 enzyme. So if you take them together, lonafarnib could slow the breakdown of modafinil and let it build up higher than expected. That might mean stronger stimulant effects, like trouble sleeping, a racing heart, jitteriness, headache, or anxiety.

To be honest, this is a theoretical concern based on how these drugs work, not on real-world reports, so I don't want to overstate it. Still, adrafinil is unregulated and hard on the liver, and lonafarnib is a serious medication. Please check with your doctor or pharmacist before combining them.

Here's the short version: Adrafinil turns into modafinil in your body, and lab studies hint that modafinil may slightly slow down a liver enzyme called CYP2C9. Your medication Lonafarnib (Zokinvy) is partly cleared by that same enzyme. So in theory, pairing the two could let Lonafarnib build up a bit higher than usual, which might mean more of its typical side effects like nausea, vomiting, diarrhea, poor appetite, fatigue, or changes seen on lab work.

I want to be honest about how soft this is. This warning is theoretical, based on a test-tube finding, not real patient cases. Still, Lonafarnib is a serious medicine used for a rare condition, so don't add Adrafinil on your own. Please run it by your prescriber or pharmacist first.

Here's the situation with these two. Your body turns adrafinil into modafinil, and modafinil can act as a mild booster of an enzyme called CYP3A4. That same enzyme is the main one that breaks down Lonafarnib (Zokinvy). So in theory, speeding up that enzyme could clear lonafarnib out of your system a little faster, which may leave you with lower drug levels and less of the benefit you're counting on.

I want to be honest that this is only a theoretical concern based on how the drugs work, not something proven in studies. Still, lonafarnib is a serious, carefully dosed medicine, so I wouldn't combine it with adrafinil without checking first. Please run it by your doctor or pharmacist before mixing the two.

About Lonafarnib

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
