Here's the gist for Nuplazid (pimavanserin): your liver leans on an enzyme called CYP3A4 to process this medicine. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. Speeding up the cleanup crew means your pimavanserin could get broken down a little faster, which may leave less of it working to control the hallucinations and delusions it was prescribed for.

I want to be honest about the strength here: this is a theoretical concern based on how these drugs behave, not something proven in real patients, and modafinil is only a weak inducer. Still, since pimavanserin is the one keeping symptoms in check, I'd loop in your doctor or pharmacist before combining them rather than changing anything yourself.
