Ashwagandha and Amlodipine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Amlodipine falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Amlodipine

Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: amlodipine already lowers your blood pressure by relaxing your blood vessels. Ashwagandha may nudge blood pressure down a little too, so stacking the two could add up and push your numbers lower than you want. If that happens, you might feel dizzy, lightheaded, tired, or a bit wobbly when you stand up quickly.

I want to be honest about the evidence, though. This concern comes mostly from animal studies, not solid human research, so it's more of a theoretical caution than a proven problem. Amlodipine isn't a prodrug, so this isn't about your liver activating it. If you'd like to try ashwagandha, it's a good idea to check in with your pharmacist or doctor first and keep an eye on your blood pressure.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body uses an enzyme called CYP3A4 to clear amlodipine. Some early lab research suggests ashwagandha may speed that enzyme up a little. If that happened in real life, your body could break down amlodipine faster, which might make it a touch less effective at controlling your blood pressure or chest pain.

Now the reassuring part. This is rated minor and is only theoretical, based on test-tube studies rather than people. Most folks won't notice anything. Still, since amlodipine keeps your numbers in check, it's smart to keep an eye on your blood pressure and watch for symptoms like dizziness or chest pain creeping back. Check with your pharmacist or doctor before adding ashwagandha so they can guide you.
