Here's the thing to keep in mind with Avacopan (Tavneos): it's one of those medications that can occasionally stress the liver on its own. Because of that, your care team usually checks your liver enzymes before you start it and along the way. Ashwagandha has also been tied to liver problems in some reported cases, ranging from mild irritation to, rarely, serious liver injury.

So the concern here isn't about ashwagandha changing how Avacopan works. It's that two things that can each be hard on the liver may add up. Watch for signs like yellowing of your skin or eyes, dark urine, belly pain, or unusual tiredness. This is a theoretical concern, not a proven one, but I'd loop in your pharmacist or doctor before combining them.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Here's the gist for Avacopan (Tavneos). Your body relies on an enzyme called CYP3A4 to break this medicine down, and lab tests hint that ashwagandha may speed that enzyme up a bit. If that happened in real life, your body could clear Avacopan a little faster, which in theory might leave less of it working to calm the immune attack on your blood vessels. For a serious condition like granulomatosis with polyangiitis, you want this drug doing its full job.

The good news: this is only a theoretical concern from test-tube research, rated minor, so it may not matter much in practice. Still, since Avacopan is a key part of your treatment, please check with your pharmacist or doctor before adding ashwagandha.

What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
