Ashwagandha and Chlorpheniramine, Phenylephrine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Chlorpheniramine, Phenylephrine falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Chlorpheniramine, Phenylephrine

Chlorpheniramine, Phenylephrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for your Histatab Plus (chlorpheniramine and phenylephrine). In lab studies, ashwagandha seems to speed up an enzyme called CYP3A4, which helps clear certain medicines from the body. In theory, that could mean a bit less chlorpheniramine and phenylephrine hanging around, so your cold and allergy symptoms (runny nose, sneezing, stuffiness) might not get covered quite as well.

Honestly, this is a minor concern. The evidence is only from test-tube and animal work, not from real people, so I wouldn't lose sleep over it. Neither of these is a prodrug that needs activating, so the effect would lean toward slightly weaker relief, not more side effects. Still, mention it to your pharmacist so they can keep an eye on things.

Serotonergic Drugs

2 of 2

Chlorpheniramine, Phenylephrine also falls under Serotonergic Drugs.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the simple version: the medicine you're asking about combines chlorpheniramine (an antihistamine) with phenylephrine (a decongestant). Chlorpheniramine has mild effects on serotonin in the body, and ashwagandha may gently nudge serotonin pathways too, based mostly on animal research. On paper, taking them together could add up a little.

Honestly, though, this is a minor, mostly theoretical concern. There are no real-world reports of problems from this combo. What I'd watch for instead is overlapping drowsiness from chlorpheniramine and ashwagandha's calming effect, so you might feel more sleepy than usual. Since phenylephrine can also raise blood pressure and cause jitteriness, just check in with your pharmacist if you take these regularly or feel off.

From our pharmacist’s overview on the Chlorpheniramine, Phenylephrine page

“A medicine that stimulates the heart and dries the body doesn't leave much room for supplements pulling the same way, and that describes chlorpheniramine, phenylephrine (Histatab Plus, Ed A Hist, Rynatan) well. Dozens of stimulant herbs, everyday caffeine sources among them, add to the decongestant's strain on blood pressure and heart…”
