Here's the gist for your cough combo. The dextromethorphan part is partly handled by a liver enzyme called CYP3A4, and very early lab research hints that ashwagandha may speed that enzyme up. If that happened in real life, your body could clear the dextromethorphan a little faster, which might make it slightly less effective at quieting a cough. The guaifenesin (the part that loosens mucus) isn't really affected this way.

Honestly, this is theoretical and based only on test-tube studies, so the real-world effect is likely small, especially for a short-term cough product. Still, if your cough isn't easing up while taking both, mention it to your pharmacist or doctor so they can help.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the deal with combining ashwagandha and a cough product like Sudal DM. The ingredient that matters here is dextromethorphan, the cough suppressant. It has some effect on serotonin in the body, and ashwagandha may gently nudge serotonin pathways too (the guaifenesin part, which just loosens mucus, isn't involved). On paper, stacking two things that touch serotonin could in theory raise the risk of too much serotonin.

That said, I want to keep this in perspective. This concern comes mostly from animal research, and there are no real-world reports of problems from this combo. The risk is rated minor. Still, if you ever notice agitation, a racing heart, sweating, shivering, or muscle twitching, stop and call your pharmacist or doctor.
