Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.

Here's the concern with pairing these two: Fruquintinib (Fruzaqla) is a cancer medication that can be tough on the liver on its own, and your care team likely checks your liver enzymes with regular blood tests while you're on it. Ashwagandha has been tied to rare but real cases of liver injury, ranging from hepatitis to serious liver failure. Stacking something that stresses the liver on top of a drug that already does can, in theory, raise the odds of trouble.

To be fair, this is a theoretical caution based on how each affects the liver, not proven cases of the two together. Still, with a serious drug like this, I wouldn't add ashwagandha on your own. Watch for yellowing skin or eyes, dark urine, or belly pain, and please loop in your oncologist or pharmacist first.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the gist for these two together. Fruquintinib is a targeted cancer medicine (used for certain advanced colorectal cancers) that your liver partly clears using an enzyme called CYP3A4. Ashwagandha isn't a prodrug situation here, so this is straightforward: some early lab research suggests ashwagandha may speed up that enzyme. If it did, your body might break down fruquintinib a little faster, which could theoretically lower its levels and make it work less well against the cancer.

Honestly, this is a very weak, theoretical concern. It comes only from test-tube studies, not from people, and it's rated a minor interaction. Still, since fruquintinib is a serious cancer drug where keeping steady levels matters, please loop in your oncologist or pharmacist before adding ashwagandha rather than starting it on your own.

What Is Fruquintinib Used For?

Brand name: Fruzaqla

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.

Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
