Ashwagandha and nilotinib

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Ashwagandha interacts with whole categories of medication. nilotinib falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

IMMUNOSUPPRESSANTS

1 of 3

Why this applies to nilotinib

nilotinib falls under IMMUNOSUPPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might decrease the effects of immunosuppressants.
Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for nilotinib (Tasigna). This medicine is a targeted cancer drug that quiets overactive signals driving chronic myeloid leukemia. Ashwagandha, on the other hand, has shown signs in lab and animal studies of revving up the immune system. The worry on file is that ashwagandha might work against the calming, treatment effect you're after, though I want to be honest: this is theoretical and based on animal data, not on real patients taking nilotinib.

So I wouldn't panic, but I also wouldn't add it on your own while you're being treated for a serious condition like CML. Please loop in your oncologist or pharmacist first so they can weigh it for your situation and keep your blood counts and response on track.

HEPATOTOXIC DRUGS

2 of 3

nilotinib also falls under HEPATOTOXIC DRUGS.

Theoretically, taking ashwagandha with hepatotoxic drugs might increase the risk of liver damage.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing ashwagandha with nilotinib (Tasigna): both can be tough on the liver. Nilotinib already carries a known risk of liver strain, and your oncology team likely checks your liver blood tests regularly while you're on it. Ashwagandha, in supplement doses, has been tied to rare but real cases of liver injury too. Stacking two things that stress the same organ is the worry here.

To be honest, this is a theoretical caution based on how each one behaves, not proven cases of the combination. Still, watch for signs like yellowing of the eyes or skin, dark urine, belly pain, or unusual tiredness. Please don't add ashwagandha without first checking with your cancer doctor or pharmacist, since they're already monitoring your liver closely.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

nilotinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this one. Your body uses an enzyme called CYP3A4 to clear nilotinib (Tasigna) out of your system. Some lab studies suggest ashwagandha may speed that enzyme up. If that happened in real life, it could push nilotinib out faster and lower its levels, which in theory might make your leukemia treatment less effective. That matters because nilotinib needs to stay at a steady level to do its job against CML.

I want to be honest about how thin this evidence is. It comes only from test-tube research, not from people taking both together, so we really don't know if it happens in the body. Still, because nilotinib is such an important cancer medicine, please don't add ashwagandha without first checking with your oncologist or pharmacist.

About Nilotinib

What Is Nilotinib Used For?

Brand name: Tasigna

  • Nilotinib is used to treat certain types of chronic myeloid leukemia (CML; a type of cancer of the white blood cells) who have recently found to have this condition in adults and children 1 year of age and older. It is also used to treat certain types of CML in adults whose disease could not be treated successfully with imatinib (Gleevec) or adults who cannot take imatinib because of side effects. Nilotinib is also used to treat certain types of CML in children 1 year of age or older whose disease could not be treated successfully with other tyrosine kinase inhibitor therapies or who cannot take these medications because of side effects. Nilotinib is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps to stop or slow the spread of cancer cells.
