Nirmatrelvir, Ritonavir falls under HEPATOTOXIC DRUGS — the group Ashwagandha interacts with.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing ashwagandha and Paxlovid: both can be tough on the liver. Paxlovid is usually taken as a short five-day course, and the ritonavir part especially has been tied to liver irritation in some people. Ashwagandha, while generally well tolerated, has been linked in rare reports to liver injury ranging from mild hepatitis to more serious problems.
So the worry is simply doubling up two things that stress the liver at the same time. Watch for warning signs like yellowing of your eyes or skin, dark urine, belly pain on the upper right side, nausea, or unusual tiredness. To be clear, this is a theoretical caution based on how these work, not proven for this combo. Since Paxlovid is short-term anyway, it's reasonable to ask your pharmacist whether to pause the ashwagandha during treatment.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with Paxlovid (nirmatrelvir plus ritonavir). Lab studies hint that ashwagandha might speed up a liver enzyme called CYP3A4, which helps clear many drugs. If that happened in real life, it could lower the amount of medicine in your blood and make the antiviral work a little less well against COVID. That matters because Paxlovid is taken as a short, full course and you want it pulling its full weight.
Now, the honest part: this is only a theory from test-tube research, not from people, so I wouldn't lose sleep over it. Paxlovid is also only a 5-day treatment, so the overlap is brief. Still, since ritonavir itself interacts with so many things, I'd run any supplement past your pharmacist or doctor before combining them.
