Nirogacestat falls under HEPATOTOXIC DRUGS — the group Ashwagandha interacts with.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing these two: Nirogacestat (Ogsiveo) can be tough on the liver on its own, and it's common to see liver enzyme changes on lab work while taking it. Ashwagandha has been tied, in rare reports, to serious liver problems, ranging from hepatitis all the way up to liver failure. So the worry isn't about one changing how the other is broken down. It's that both can stress the liver, and stacking them may add to that risk.
I'll be honest, this is a theoretical caution based on how each behaves, not proven cases of the combination. Still, since you're already on a drug that needs liver monitoring, I'd loop in your oncology team or pharmacist before adding ashwagandha. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness.

In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two. Nirogacestat (Ogsiveo) is a cancer medicine used for desmoid tumors, and your body clears it using an enzyme called CYP3A4. Ashwagandha, at least in lab studies, seems to speed up that same enzyme. In theory, that could push nirogacestat out of your system a little faster, which might mean less of the drug is available to do its job.
I want to be honest about how weak this signal is. This concern comes only from test-tube research, not from people taking both together, so it's flagged as minor and theoretical. Still, nirogacestat is a serious cancer treatment where you don't want to risk it working less well. Before adding ashwagandha, please check with your oncologist or pharmacist first.
