Pemigatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Here's the gist for Pemigatinib (Pemazyre): your body relies on a liver enzyme called CYP3A4 to help process this medicine. Lab studies (test tube only) hint that ashwagandha might speed that enzyme up a bit. In theory, a faster enzyme could clear Pemigatinib out of your system a little quicker, which could mean it works less well against your cancer.

I want to be honest that this is a very weak, theoretical concern. We don't have human data showing it actually happens. Still, because Pemigatinib is an important cancer treatment where keeping the right level matters, I'd play it safe. Please check with your oncologist or pharmacist before adding ashwagandha, rather than starting or stopping anything on your own.

About Pemigatinib

What Is Pemigatinib Used For?

Brand name: Pemazyre

Pemazyre (pemigatinib) is used to treat adults with a type of bile duct cancer called cholangiocarcinoma that cannot be surgically removed (locally advanced or metastatic) and has already been treated with at least one prior therapy. It is only for tumors that carry a specific gene change — an FGFR2 fusion or rearrangement — confirmed by an FDA-approved test. This approval is based on tumor response rates, and further studies are underway to confirm long-term benefit.

Pemazyre is also used to treat adults with relapsed or refractory myeloid/lymphoid neoplasms (MLNs) — blood or lymph system cancers that have stopped responding to or come back after prior treatment — when those cancers carry an FGFR1 rearrangement gene change.
