Phentermine, Topiramate falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.

Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.

Here's the thing about pairing ashwagandha with Qsymia (phentermine plus topiramate): this is a theoretical concern based on how the herb behaves, not something proven in studies. Ashwagandha can have a mild calming, sleepy effect for some people. The topiramate half of Qsymia can already cause drowsiness, dizziness, trouble concentrating, and that foggy or sluggish feeling. So in theory, stacking the two together could make those effects a little stronger.

The phentermine side is actually a stimulant, so the picture can be mixed. Realistically, for most healthy adults this is a low-level caution, not an alarm. Still, since you'd notice it most as extra grogginess, mention it to your pharmacist or doctor before starting ashwagandha, especially if you'll be driving.
