Why this applies to Pimozide
Pimozide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for pimozide (Orap): your liver uses an enzyme called CYP3A4 to clear it out. Some lab studies suggest ashwagandha may speed that enzyme up, which could in theory lower pimozide levels in your body. Since pimozide is the active drug (not a prodrug that needs activating), faster clearance might mean it works a little less well, so your Tourette's tics could become harder to control.
I want to be honest about the evidence: this is only a theoretical concern from test-tube research, not real patients, and it's rated minor. Still, pimozide is a medication that needs steady levels and careful monitoring, so I wouldn't start ashwagandha on your own. Please run it by your pharmacist or doctor first.

Pimozide tablets are used to suppress the motor tics (involuntary movements) and phonic tics (involuntary sounds or vocalizations) that occur in Tourette's Disorder. It is reserved for patients whose daily life or development is severely affected by these tics.
Pimozide is not a first-choice treatment, and it is not meant for tics that are only mildly bothersome or cosmetic in nature. It should only be considered when standard treatments have not worked well enough.
