These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Ashwagandha interacts with whole categories of medication. Remimazolam Besylate falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CNS DEPRESSANTS
1 of 2Why this applies to Remimazolam Besylate
Remimazolam Besylate falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: ashwagandha has its own mild calming, sleepy effect. Remimazolam (Byfavo) is a short-acting sedative given through an IV to relax you for procedures like colonoscopies. Putting two things that both slow down the brain together could, in theory, add up and make the sedation deeper than expected. Think more drowsiness, slower breathing, or a groggier recovery.
Now, the honest part: this concern is theoretical, based on how each one works, not on real cases. Since remimazolam is given by an anesthesia provider who monitors you closely, the bigger thing is simply telling them you take ashwagandha before any procedure. Don't stop anything on your own. Just give your care team and pharmacist the full list of your supplements.

BENZODIAZEPINES
2 of 2Remimazolam Besylate also falls under BENZODIAZEPINES.
There is preliminary evidence that ashwagandha might have an additive effect with diazepam (Valium) and clonazepam (Klonopin). This may also occur with other benzodiazepines.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two. Remimazolam (Byfavo) is a fast-acting sedative given by IV, usually during procedures like a colonoscopy, to help you relax and feel drowsy. Ashwagandha is taken to calm the nervous system and ease stress. Both can have a quieting, sleepy effect on the brain, so the concern is that together they could add up and leave you more sedated than expected.
To be fair, this is a theoretical caution based on how these things work, not proven cases with remimazolam specifically. Still, since remimazolam is given by an anesthesia team for a procedure, it's worth mentioning ashwagandha to them beforehand so they can adjust. Please don't stop either one on your own. Just let your doctor or pharmacist know what you're taking.
