Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.

Here's the simple version: ribociclib (Kisqali) can be tough on the liver on its own, and your care team already checks your liver enzymes with regular blood work while you're on it. Ashwagandha has been tied to rare but real cases of liver trouble, ranging from inflammation to, in a few reports, serious liver injury. Putting the two together could, in theory, add to that strain on your liver.

To be fair, this concern is theoretical and based on how each one behaves, not on solid studies of the combination. Still, with a cancer medicine this important, I wouldn't add ashwagandha without checking first. Watch for yellowing of your skin or eyes, dark urine, belly pain, or unusual tiredness, and please talk with your oncologist or pharmacist before starting it.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Your body clears ribociclib (Kisqali) using a liver enzyme called CYP3A4. Some lab studies suggest ashwagandha may speed that enzyme up. In theory, that could push ribociclib out of your system a little faster and slightly lower its levels, which matters because this is a cancer medicine you want working at full strength.

I want to be honest about how thin this evidence is. This is a theoretical concern based only on test-tube and animal research, not real patients, and it's rated minor. Still, because ribociclib is treating breast cancer and even small dips in drug level aren't worth gambling on, please don't add ashwagandha without first running it by your oncology team or pharmacist.

What Is Ribociclib Used For?

Brand name: Kisqali

Kisqali is used to treat adults with hormone receptor (HR)-positive, HER2-negative breast cancer. For early breast cancer (stage II or III, at high risk of recurrence), Kisqali is used together with an aromatase inhibitor as adjuvant treatment — meaning it's given after initial treatment to help reduce the chance the cancer comes back.

For advanced or metastatic breast cancer (cancer that has spread), Kisqali is used in combination with either an aromatase inhibitor or fulvestrant — either as the first hormone-based treatment or after the cancer has progressed on another hormone therapy.
