Theoretically, taking ashwagandha with hepatotoxic drugs might increase the risk of liver damage.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.

Here's the concern with pairing ashwagandha and Qinlock (ripretinib). This isn't about ashwagandha changing how much ripretinib gets into your system. Instead, it's about doubling up on things that can be tough on the liver. Ripretinib can raise liver enzymes on its own, and ashwagandha has been tied to rare but serious liver problems in a handful of case reports. Stacking the two could, in theory, add to that strain.

I'll be honest, this is a theoretical caution based on how each acts, not proven cases of them together. Still, with a cancer medicine like this, I'd play it safe. Watch for yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your oncologist or pharmacist before adding ashwagandha.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the gist for Qinlock (ripretinib). Your body relies on an enzyme called CYP3A4 to help process this cancer medicine. Lab studies (test tube only, not people) hint that ashwagandha may speed that enzyme up a little. If that happened in real life, your body could clear ripretinib a bit faster, which in theory might leave less of it working against the tumor.

I want to be honest about how shaky this is. This is a theoretical, minor concern based only on lab data, not on actual patients. Still, with a serious cancer treatment like this, I wouldn't want anything nibbling at how well it works. Please loop in your oncology team or pharmacist before adding ashwagandha, so they can keep you on the safe side.
