Why this applies to Vorapaxar
Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for vorapaxar (Zontivity). Your body clears this blood thinner using a liver enzyme called CYP3A4. Lab studies hint that ashwagandha may speed up that enzyme, which could lower vorapaxar levels and make it a bit less effective at keeping platelets from clumping. Since vorapaxar is meant to lower your risk of another heart attack or stroke, a weaker effect is the concern here, not extra bleeding.
That said, this is rated minor and rests only on test-tube research, not real patients, so I wouldn't lose sleep over it. Still, vorapaxar is serious heart medicine, so please check with your pharmacist or doctor before adding ashwagandha rather than starting it on your own.

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.
Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
