Phenobarbital, Phenytoin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might increase levels of drugs metabolized by CYP2C19 enzymes.
In vitro evidence suggest that wogonin, a constituent of Baikal skullcap, modestly inhibits the activity of CYP2C19 enzymes. This effect has not been reported in humans.

Here's the gist with this combo. Both phenytoin (Dilantin) and phenobarbital are partly cleared by a liver enzyme called CYP2C19. A compound in Baikal skullcap, wogonin, has shown in lab tests that it can mildly slow that enzyme down. If that happened in your body, these seizure medicines could clear out a little slower and edge upward, possibly bringing on more side effects like drowsiness, dizziness, unsteady walking, slurred speech, or confusion.

I want to be honest about the strength of this, though. It's only been seen in test-tube studies, not in actual people, so it's theoretical. Still, phenytoin in particular is a narrow-window drug where small shifts matter, so please check with your pharmacist or doctor before adding this herb.

Theoretically, Baikal skullcap might cause additive therapeutic and adverse effects when used concomitantly with drugs with sedative properties.
In vitro and animal research suggests that Baikal skullcap binds to GABA-A receptors and causes sedation. Theoretically, Baikal skullcap might cause additive therapeutic and adverse effects when used concomitantly with drugs with sedative properties. Preliminary clinical research has not identified clinically relevant sedation after use of Baikal skullcap; however, a thorough evaluation of safety outcomes has not been conducted.

Here's the gist with this combination. Baikal skullcap has been shown, mostly in lab dishes and animal studies, to gently nudge the same calming GABA receptors in the brain that a sedative like phenobarbital works on. In theory, taking them together could add up to a little extra drowsiness, so you might feel more tired, foggy, or unsteady on your feet than usual. Phenytoin can already cause some of that sleepiness too.

That said, this is a very weak, mostly theoretical concern. Early human research hasn't actually shown meaningful added sedation from the herb. Still, since phenobarbital is a true sedative and phenytoin needs steady levels to control seizures, I'd run any new supplement by your pharmacist or doctor first.
