Your medicine combines two blood pressure lowering drugs in one pill: diltiazem (a calcium channel blocker) and enalapril (an ACE inhibitor). The concern here is simple stacking. Betony may have a mild blood pressure lowering effect of its own, so adding it on top of a medicine that already drops your pressure could push it down a little too far.
If that happens, you might feel dizzy, lightheaded, tired, or notice your heart pounding, especially when you stand up quickly. Keep in mind this is a theoretical interaction based on how betony's plant compounds behave, not on solid human studies, so for most people it stays mild. Still, it's worth a quick chat with your pharmacist or doctor before adding betony, and checking your pressure at home can help.
