PHENYTOIN (Dilantin)
Black tea contains caffeine. Animal research suggests that caffeine can decrease the anticonvulsant activity of phenytoin. The effect does not seem to be related to the seizure threshold-lowering effects of caffeine. However, the exact mechanism of this interaction is unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for anyone taking mephenytoin (Mesantoin) alongside black tea. Mephenytoin is an anticonvulsant in the same family as phenytoin, used to help prevent seizures. The concern on file is that the caffeine in black tea might weaken how well this medicine controls seizures, which could in theory raise the risk of a breakthrough seizure.
I want to be honest about the evidence: this comes only from lab and animal studies, and the exact reason isn't well understood. It's a theoretical, low-level concern, not a proven problem in people. A cup or two of tea is unlikely to be a big deal for most folks, but since seizure medicines leave little room for error, run your tea or caffeine habits by your pharmacist or doctor.
